The Behavioral Health Log feature enables authorized behavioral health providers to log and manage behavioral health entries in a centralized and secure environment. Integrated with our existing Health Log, this new feature is specifically designed for social workers and counselors to ensure a seamless and holistic approach to child healthcare and wellbeing.

Provider Permissions
With the addition of the Behavioral Health Log feature, there are two sets of Health Log permissions, allowing primary providers to create two distinct roles:
- Health Log: General
- Health Log: Behavioral
Important: Access to the Health Log: Behavioral is not available by default. As shown in the screenshot below, the permissions for the Health Log: Behavioral are set as None by default. View or Edit permissions for the Behavioral Health Log will need to be manually added to any roles that require access to this feature.
To ensure confidentiality and compliance, organizations are encouraged to set custom permissions to limit the Behavioral Health Log feature to only authorized behavioral health staff.
Important: A provider with access to the Behavioral Health Log, and who is registered to view the associated participant, can access any Behavioral Log entries. These entries are not restricted to the provider who originally created them.
In the event that General Health providers will need access to the Behavioral Health Log, their permissions will need to be adjusted. For more information on managing permissions, please check out the Permissions support article.
Behavioral Health Log Entries
Like General Health Log entries, Behavioral Health Log entries follow the standard SOAP note format (an acronym for Subjective, Objective, Assessment, and Plan), a widely adopted communication tool between interdisciplinary healthcare providers.
Our application allows you to document a participant’s behavioral health-specific chief complaint, history, assessment/plan, and other items. Additionally, the entry options are designed to efficiently record entries by including session time and encounter type.
Entries in black signify a completed entry, while entries in red indicate a draft that has not yet been submitted. You may filter through these entries using conditions, which work in a similar fashion as the List Builder.
Just like the General Health Log, the Behavioral Health Log is tamper-proof. This means that entries cannot be deleted, and they cannot be altered by anyone other than the provider who originally entered them. Entries can have an addendum and linked visits added by other providers, but the original entry will always remain intact. Entries can also be marked as reviewed.
Create New Behavioral Health Log Entry
Before you begin creating Behavioral Health Log entries, you can customize the locations available in the Health Log. Check out our Settings article to learn more.
When you create a new Health Log entry, you can add additional fields by selecting items from the Objective, Assessment/Plan, and Additional categories to inform the final assessment. Drafts automatically save as you type, and you can come back to drafts to make edits at any point in time.
To create a new Behavioral Health Log entry:
- In the Menu, click Health Log.
- In the submenu that appears, click New.

In the Log Type dropdown, select Behavioral Log.
Note: If you are creating a new General Health Log entry, select General Log. - In the Participant dropdown, type the name of the participant, and then select the name from the list that appears.
Important: You can only create entries for the dates that fall within the time that a participant is registered to attend a session. If you try to create an entry during a date outside of these times, you will receive an error message that says, "Participant has no active registrations at the time selected above." - (Optional) If you want to change the Date and Time of the Health Log entry, click the Logged dropdowns and select the correct date and time.
Note: The date and time defaults to the current date and time. - (Optional) If you want to use a template for the entry, click the Load a Template dropdown, and then select the template that you want. Learn how to create templates in the Health Log Templates article.
Tip: Behavioral Health Log templates include fields for Session Time and Encounter Type. - Complete the information in all of the boxes outlined in red. These are required fields, and you can hover your cursor over the Tool Tip icon () to see a description of the type of information needed.
- (Optional) Click the fields found within the Objective, Assessment/Plan, and Additional categories in the right column to add fields to the Behavioral Health Log form.
Tip: Some of the fields give you the option to add more than once response.
Tip: In the Assessment/Plan section, click the Follow-Up field to set a News Feed alert to follow up with the participant. - (Optional) If you want to upload a document, click the File box and select the document or drag and drop the document in the box.
Note: Due to the sensitive nature of behavioral health-specific documentation, we encourage providers to only upload documentation relevant to the encounter and on an as-needed basis. - (Optional) If you want the authorized users on a participant's profile to receive an email summary of the Health Log entry, click the Do NOT Notify dropdown, and then select Notify Authorized Users.
Note: By default, authorized users cannot view and are not notified about Health Log entries. - Do one of the following, depending on whether you want to submit the Behavioral Health Log entry, submit the entry and begin a new entry, or delete the entry draft:
- If you want to submit the Behavioral Health Log entry, click the SUBMIT button.
- If you want to submit the Behavioral Health Log entry and begin a new entry, click the SUBMIT AND NEW button.
- If you want to delete the Behavioral Health Log entry draft, click the DELETE DRAFT button.
Important: A Behavioral Health Log entry cannot be deleted after it has been submitted. You can mark it as an error if there is a mistake with a submitted entry. Check out the Health Log Updates article to learn more.
