Medication Confirmation Forms
Greetings! I swear last camp season when I pulled the Medication Confirm Form report, it included a section for the healthcare provider to sign off on. Now it only says "Parent/Guardian Authorization". Am I going crazy or am I missing something in the report options?! Our medical team is looking for a way to capture more accurate medication information beyond parent's self-report in the health profile (sometimes this is not accurate or up to date).

The Medication Confirmation Form report does include a place for the Healthcare Provider to sign an authorization for the medication, Madeleine. You can only see this authorization section in a form for a camper with a current registration and an active medication. I included an image from a Medication Confirmation Form I generated for the Test, Camper profile in your account. Is this what you're looking for?

You can filter for your list of campers in the List Builder, create the Standard Medication Confirmation Report, and then use the Send Message feature to email the form to caregivers.
As a potentially easier alternative, you can add a Medication Confirmation Form to the Health Profile you ask your campers to complete. You can talk to your Client Success Pod if this is something you're interested in!
